This questionnaire asks for certain confidential information as a means for us to perform initial due diligence to determine whether the Environment Control franchise program may be beneficial for you. There is no single question in this questionnaire that would disqualify you from owning an Environment Control franchise. We are trying to assist your own analysis in making the right decision for your future, so please answer each question truthfully and completely. Each individual who anticipates holding an equity position in the franchise should fill out a separate application. Since all Environment Control franchises are independently owned and operated, it benefits every applicant (as early in the process as possible) to analyze their suitability for ownership of an Environment Control franchise. We will maintain all of this information as confidential and will not disclose it or use it in any manner other than to facilitate our analysis of you as a potential Environment Control franchise owner.
